Hello,

You can find reviews of the Youth Outcome Questionnaire (Y-OQ-2.01) in the Mental Measurements Yearbook database.  It is available through the library's research guide on tests and measurements at http://libguides.unomaha.edu/tam

The instrument itself is not available through any of the databases.  It is a commercially published instrument available from the company OQ Measures at http://www.oqmeasures.com/measures/youth-adolescent-measures/

Normallly, you would have to purchase it.  If you need to just look at it, you might call the company to see if they have student rates.  Or, there is a test library in  Lincoln called the Buros Institute.  They publish the database I mentioned above and have a test library at the UNL campus.  You could call and ask if you could look at the instrument.  Their number is  (402) 472-6203
