The instrument itself is not available through any of the databases.  It is a commercially published instrument available from the company OQ Measures at

Normallly, you would have to purchase it.  If you need to just look at it, you might call the company to see if they have student rates.  Or, there is a test library in  Lincoln called the Buros Institute.  They publish the database I mentioned above and have a test library at the UNL campus.  You could call and ask if you could look at the instrument.  Their number is  (402) 472-6203
